Operator decides; ratchet records. Includes a dependency-graph ASCII diagram, explicit deferral list (EngineIdentity shelved, L4-L9 findings pending, recognizer-drop-off ADR derived from named items), and a "how the ratchet evolves" section codifying append-only revision discipline.
